CSLB Application Process: Step-by-Step (Form 13A-1)
The CSLB application is Form 13A-1 (Application for Original Contractor's License). It's 12 pages, 9 sections, and the single most common failure point in the whole licensing process. About 30% of applications are returned for corrections on the first pass. Here's how to submit a clean one.
Section 1-3: applicant info
Legal name, DOB, SSN/ITIN, contact info, entity type (sole prop, partnership, corp, LLC). Legal name must exactly match your ID — no nicknames.
Section 4: classification
One classification per application. If you want multiple, submit each separately or add later via Form 13A-3.
Section 5: qualifying individual (QI)
Who's taking the trade exam. Owner, RME, or RMO. RMOs must own 10%+ of the business or post an additional bond.
Section 6: experience certification
Every year of experience must be signed by a qualifier. Qualifier name, license number, phone, and relationship. CSLB does call these references.
Section 7: criminal history
Disclose every conviction (not arrests). Failure to disclose is grounds for denial even for minor misdemeanors that would otherwise be waived.
Section 8-9: fees and certification
$450 application fee (non-refundable) plus signature under penalty of perjury. Include Live Scan receipt copy.

Use CSLB's online Application Status Check with your file number (mailed 2-3 weeks after submission). Typical timeline: 8-12 weeks to Exam Eligibility Letter.
